   3. Minimum 1 year as an orthopedic surgeon
   4. Maximum age is 40 years old at start the education

   6. Fill SOSSMA fellowship form registration
   7. Letter of recommendation
      1. one letter from PABOI members
      2. one letter from SOSSMA faculty members
      3. one letter from prospective participant workplace
   8. Active registration certificate (STR)
   9. Letter statement of willingness to follow SOSSMA fellowship
   10. Photos 3x4 cm (4) and 4x6 cm (4)
